How to Request Certificates

Identify Your Needs

Determine how many membership certificates are needed before beginning your request.

Fill in all required fields in the form. Incomplete requests may cause delays.

Allow Processing Time

Certificate requests are reviewed and processed by the Council office. You will be contacted once your documentation is ready for issuance or delivery.

Certificates are $20.00 per certificate.

Important Notes

Please submit only one request per temple. All individuals who need certificates must be included in a single submission. Requests must be submitted by an authorized temple officer.
